US Medical Licensing: State Jurisdiction and Board Certification The United States, functioning as a federation, accords significant legislative rights to its constituent States. Consequently, the issuance of a US Medical License falls within the purview of individual States. Navigating a Medical Career in the US involves two distinct processes—Medical Licensing and Board Certification. Board Certification, notably the Education Commission for Foreign Medical Graduates (ECFMG) Certification, constitutes a separate procedure entirely. The ECFMG operates on a centralized system, meticulously assessing a physician's knowledge and skills through an elaborate certification process. However, even with Board Certification in hand, a physician in the US can practice only upon receiving an American Medical license from a State. While there are federal (central) positions filled by medical personnel—such as those in US Military or Federal prisons—these doctors too require licensure from one of the US States. Typically, a physician practices within the State that grants them a State Medical License, which serves as the primary medical credential in the USA. Moreover, the contemporary surge in Information Technology has given rise to the advent of "Tele-Medicine," enabling physicians to diagnose and treat patients across long distances. However, certain states impose restrictions on this practice as well. A Glimpse into the History of US Medical Licensing In 1791, the States were granted the authority to regulate health, as stipulated in the Bill of Rights. Historical records reveal that until the 1870s, the majority of physicians in the country practiced without a US Medical License, with only a few exceptions. It was not until 1877, spurred by the State of Illinois's initiative, that many States began implementing laws to regulate medical practice within their jurisdictions. | |
